Five Phase Wellness Center’s Anniversary Open House Celebration!

We are thrilled to be an established health care and wellness resource for the Evanston & surrounding communities! Come celebrate our 1-Year Anniversary with us! We are hosting this special event for our clients, families, old and new friends to thank you for your support during our first year of business. Raffles will be held for your chance to win free services. We will also be introducing new programs. Learn how we can help you achieve your goals as you begin or enhance your wellness path to look and feel your very best. Light refreshments will be served. All are welcome!
